Acute Coronary Syndrome In a Patient with Previously Undescribed Coronary Artery Anomaly Captain Andrea M. Porrovecchio, ACP Associate, Lieutenant Colonel Michael A. DeGeare, Fellow ACP, Keesler Medical Center, Keesler AFB, Mississippi Case: A 74-year-old female with a history of CAD presented to the emergency room with a two-month history of worsening neck tightness, dyspnea, nausea and diaphoresis. Her left main coronary artery gave rise to only a left anterior descending coronary artery. The right coronary artery was selectively engaged and found to be small and non-dominant. However, a dominant left circumflex artery was noted to arise from the proximal RCA. A 80-90% stenosis was seen in the LCX and successful PTCA was performed There was inadequate guide support using a 6F system to allow successful stenting. ; At the conclusion of the procedure the patient had TIMI 3 flow and was chest pain free but was noted to have a type B dissection and a residual 30% stenosis. She was transferred to the CCU with continuation of her IIB IIIa blockage. She later developed recurrent chest pain with ECG changes and the anomalous LCX lesion was successfully stented using an 8F R1 guide. Discussion: This is the first known description of an anomalous dominant left circumflex artery arising from a non-dominant right coronary artery. Coronary artery anomalies in general are rare with a 1% incidence in coronary angiography and a 3% incidence at autopsy. Most are minor, benign variations that are seldom clinically significant. The spectrum of symptoms in clinically significant coronary anomalies includes angina, syncope, CHF, MI and sudden cardiac death. Certain anomalies, especially those which course between the aorta and pulmonary artery, are believed to cause reduced myocardial blood flow. There is no known association with early atherosclerosis and it is unlikely that our patient's acute coronary syndrome was related to her coronary anomaly.
